Children's Advanced Social Work Practitioner

This is an exciting time to join Sheffield as an Advanced Social Work Practitioner, in the Leaving Care Service. You would be expected to hold a reduced complex Case Load whilst offering Management Support to the Senior Fieldwork Managers.

You will:

• Work autonomously to provide a high-quality social work service to children, their families and carers by using relationship based practice.
• Deliver a social care service to families with a high level of social complexity and risk of harm to children and young people, following service standards and the statutory framework.
• Work in partnership with children, their families, carers and professionals to identify the right services for children who need help and protection or who need support to progress whilst in care or leaving care. Maintain accurate, case notes within statutory timescales, prepare plans, reviews and reports and carry out statutory supervision.
• Support young people in transition, including moving to and between placements, returning to family, or moving to independence.
• Mentor and support Personal Advisors, so they can achieve excellent practice and decision making for complex, high risk cases and oversee social work students on placements.
• Be accountable for ensuring the highest professional standards, conduct and quality of social work practice.
