We are recruiting for a Senior Practitioner Post in Swift Response Team.
We are looking for candidates that recognise the value of multidisciplinary working and promote a whole service approach, with our internal & external colleagues.
SRT work is fast paced including Supported conversations, Care Act assessments and a good knowledge of local services, including tier 1 & 2.
You will have an interest in, and experience of, working in a multi-disciplinary way, be able to work quickly and proportionately and have confidence in respect of decision making and positive risk taking.
You will ensure citizens have the right professional expertise for their presenting needs at the earliest opportunity: preventing or delaying the need for care and support through maximising independence, autonomy, community engagement, managing or enabling risk, and health promotion.
The Senior Practitioner role involves supervising social care practitioners, social workers, and overseeing the development of ASYEs & student social workers; being part of the leadership team and undertaking strategic work; and building relationships with internal and external partners.
You will need to be a good communicator, with excellent time management skills. A social work or occupational therapy qualification and registration with relevant bodies is essential.

Swift Response Team
Centrally located with other adult social care teams in City Hall, with great transport links across the city. The team is a large multidisciplinary team made up of Social Workers, Social Care Practitioners, Occupational Therapists, OT aides and Care Direct Advisors. The main function of the team is to be the front door of adult care, triaging and respond to urgent requests for support citywide. The team have a wealth of knowledge of Tier 1 and 2 services and respond quickly to referrals from professionals and citizens.
You will support practitioners in team to triage referrals, complete supported conversations, respond to urgent requests for support, undertake care act assessments and Occupational therapy assessments.
